The initial inquiry report into the PIMS tragedy in Islamabad has been submitted to Prime Minister Shehbaz Sharif, with recommendations for action against those found responsible for negligence or misconduct.
According to sources, the inquiry committee formed to investigate the incident has completed its report and submitted it to the Prime Minister’s Office.
The report was submitted by former Interior Secretary Shahid Khan, who headed the inquiry committee, at the office of Prime Minister’s Adviser Tauqeer Shah.
Sources said the report was submitted late at night, with the committee also briefing the relevant officials about its key findings and recommendations.
All Inquiry Committee Members Signed the Report
According to sources, the report carries the signatures of all members of the inquiry committee.
The committee examined several aspects of the PIMS tragedy, including the circumstances surrounding the incident, its causes, administrative arrangements and the roles of officials and staff members.
The report also includes observations regarding the situation during the incident and the response and rescue operations that followed.
Action Recommended Against Absent Staff
Sources said the inquiry report recommends action against relevant PIMS employees who were absent from duty at the time of the tragedy.
The committee examined staff attendance, the responsibilities assigned to employees and whether those responsibilities were properly fulfilled during the incident.
After determining responsibility, the committee recommended that individuals found guilty of negligence or dereliction of duty should face action in accordance with applicable laws and regulations.
Rescue and Emergency Operations Reviewed
The inquiry committee also examined the difficulties faced by rescue teams during emergency operations.
According to sources, statements from the fire brigade and other rescue teams were included in the report.
The committee sought to determine what difficulties emergency responders faced after the incident and what measures could be introduced to improve the response in future emergencies.
The availability of emergency equipment, administrative coordination and the procedures used to respond to the incident were also reviewed.
Questions Raised Over PIMS Facilities
The report also examines the medical facilities and other basic arrangements available at Pakistan Institute of Medical Sciences (PIMS).
Sources said the committee identified several shortcomings and described some facilities and arrangements at the hospital as inadequate.
The report reportedly noted that certain facilities and arrangements did not meet required standards, potentially creating difficulties for patients and affected individuals during major emergencies.
PIMS Administration and Doctors Interviewed
During the investigation, the inquiry committee interviewed PIMS administration officials, doctors and other relevant staff members.
According to sources, information was collected from various officers and employees to establish what happened before, during and after the incident.
The committee used these statements to assess the causes of the tragedy, administrative weaknesses and the hospital’s ability to respond effectively to emergency situations.
Recommendations to Prevent Similar Incidents
The inquiry report does not focus solely on identifying those responsible.
It also contains recommendations aimed at preventing similar incidents in the future.
Sources said the committee recommended improvements to emergency arrangements at the hospital, stronger rescue capabilities and clearer definitions of staff responsibilities.
The committee also emphasized the need for better coordination between the hospital administration, fire brigade and other emergency response organizations.
According to the recommendations, an integrated emergency response system should be established so that relevant institutions can respond quickly and effectively whenever a major incident occurs.
Prime Minister to Decide Whether Report Will Be Made Public
Sources said the decision on whether to make the inquiry report public rests with the prime minister.
The Prime Minister’s Office has been briefed on the report’s findings and recommendations. However, the prime minister will decide whether the complete report should be released publicly or whether only selected portions should be made available.
Sources said the report contains specific recommendations regarding the identification of those responsible and possible action against them.
Prime Minister Decides on Immediate Action
According to sources, Prime Minister Shehbaz Sharif has decided to take immediate action in light of the inquiry report.
Following the identification of those responsible and the recommendations presented by the committee, action against relevant individuals could be initiated soon.
Sources said the prime minister may issue directions to the concerned institutions after reviewing the findings and recommendations.
